What is Viagros (Sildenafil)?

Viagra is the best-known and most popular medication for treating the inability of the penis to achieve or maintain an erection during sexual stimulation. Sildenafil citrate is the active ingredient in Viagra. Its potent effect in treating erectile dysfunction (ED) is recognized and proven. Initially, sildenafil was researched as a remedy for angina pectoris, a condition characterized by chest pain due to inadequate blood flow to the heart. However, during clinical trials, its remarkable impact on the penis's ability to achieve or maintain an erection was discovered. This effect allowed the scientific community to approve the component as an effective treatment for erectile dysfunction.

The mechanism of action of Viagra is as follows: it improves the penis's ability to achieve or maintain an erection by increasing blood flow to the organ during sexual arousal. Manufacturers produce it in different forms, with tablets being the most popular due to their convenience. In this way, companies are able to meet the needs of all consumers.

In addition to erectile dysfunction, the ingredient demonstrates efficacy in the treatment of pulmonary arterial hypertension (PAH). Its benefit for this diagnosis is clinically proven. The component is able to dilate blood vessels, which helps reduce resistance in the pulmonary vasculature. This action improves exercise capacity and slows the progression of the disease. Currently, new research is investigating the potential effect of the ingredient on other medical conditions, suggesting a wide range of therapeutic uses.

Benefits

Treatment of Erectile Dysfunction

Firstly, Viagros acts as a potency enhancer. Being a powerful weapon in the fight against erectile dysfunction, its components improve male sexual health. Millions of men worldwide suffer from the inability to respond to sexual stimulation, which affects their libido, quality of life, and psychological well-being. By ensuring a natural erectile response to sexual stimulation, Viagros improves sexual performance and satisfaction. This effect has been proven by millions of men who have used this potency pill.

Treatment of Pulmonary Arterial Hypertension

Viagra is also effective in treating Pulmonary Arterial Hypertension (PAH). This condition is characterized by high blood pressure in the pulmonary arteries. Sildenafil helps reduce blood pressure by acting on the pulmonary blood vessels and relaxing them. In addition, the progression of the disease is slowed and the ability to exercise is improved.

Cardiovascular benefits

Increasing evidence is emerging of the effectiveness of sildenafil for cardiovascular problems. Numerous studies have proven that the component has a protective effect on the cardiovascular system. The ingredient demonstrates its effectiveness in reducing abrupt changes in oxidative processes under conditions of insufficient functioning of the antioxidant system. Furthermore, it improves the regulation of capillary blood flow through the dynamic alteration of vascular constriction and dilation phases. Finally, the medication enhances the method of supplying and transporting blood and biologically active substances through the vascular system to the body's organs and tissues.

In the graph, you can observe the maximum mean reductions in systolic (A) and diastolic (B) blood pressure in the supine position over time, induced by increasing doses of sildenafil in healthy middle-aged volunteers (n = 16 per group). The values represent the mean and standard deviation.

Improved exercise capacity

Another benefit of the ingredient is its ability to significantly increase physical performance. This advantage is due to its ability to improve blood circulation in the vessels and increase the supply of oxygen to the muscles. The increased blood flow in a closed system, caused by the pressure difference in different parts of the vascular bed, contributes to more efficient physical performance and reduced fatigue. Notable improvements in exercise tolerance provided by the substance are beneficial for athletes.

Mechanism of action

To exert its beneficial effect on the body, sildenafil acts through a complex biochemical pathway. Suppression of the phosphodiesterase type 5 (PDE5) enzyme is an important part of this process. In addition to potency problems, the medication addresses other issues that contribute to men's health.

Normal physiology of erection

Penile erection during sexual arousal is a complex neurovascular process involving neural, psychological, hormonal, and vascular factors.

  1. Sexual stimulation.  The process begins with tactile and visual contact, which triggers the release of neurotransmitters in the nerve endings of the penile tissue.

  2. Nitric oxide release.  Nerve endings are located in the corpus cavernosum. These neurotransmitters affect them and stimulate the release of nitric oxide (NO).

  3. Activation of guanylate cyclase.  NO penetrates the smooth muscle cells of the penis and activates the enzyme guanylate cyclase.

  4. cGMP production.  Under the action of guanylate cyclase, the process of converting guanosine triphosphate (GTP) into cyclic guanosine monophosphate (cGMP) is accelerated. The latter acts as a vital signaling molecule for the organism.

  5. Relaxation of smooth muscle.  cGMP causes relaxation of the smooth muscle of the penis, reducing the intracellular level of calcium. This effect results in dilation of blood vessels, which, in turn, allows an increase in blood flow to the male organ.

  6. Erection.  The corpora cavernosa fill with blood due to improved circulation, causing the penis to harden and increase in size. Because the veins are compressed, blood cannot leave the organ, allowing the erection to be maintained.

Role of PDE5

An enzyme called phosphodiesterase type 5 (PDE5) is present in the pulmonary vessels and smooth muscle cells of the corpus cavernosum. Its main function is to break down cGMP into 5′-GMP. This action leads to the interruption of the signal for relaxation of the smooth muscle of the penis. As a result, blood flow is reduced, which causes loss of erection. Sildenafil has a therapeutic effect and acts selectively on PDE5, which allows for increased treatment efficacy. The drug's mechanism of action includes the following phases:

  1. Interaction with PDE5.  When the ingredient enters the body, it targets PDE5. By binding to it, Sildenafil acts selectively, preventing PDE5 from interacting with and degrading cGMP.

  2. Prolonged duration of cGMP action.  Because the drug suppresses the action of PDE5, the cGMP level remains elevated for a longer period, prolonging the relaxation of smooth muscle cells.

  3. Increased vasodilation.  Keeping the smooth muscle relaxed prolongs the dilation of blood vessels, allowing more blood to enter the penile tissue.

  4. Prolonged erection  . A stable and sufficient blood flow to the male sexual organ ensures the erection is maintained for longer.

Benefits for pulmonary and systemic circulation.

Although the ingredient is primarily used for potency treatment, it can still affect the pulmonary vasculature due to its effect on PDE5. The following benefits are observed:

  1. Relaxation of pulmonary vessels.  By suppressing PDE5 activity, the component relaxes pulmonary vessels, leading to their widening and improved bandwidth. This action reduces pulmonary vascular pressure and resistance, which is beneficial in conditions such as pulmonary arterial hypertension (PAH).

  2. Systemic circulation.  By acting on systemic vessels, the drug may cause a slight reduction in systemic blood pressure. Although this effect is relatively modest, it can become significant in patients with cardiovascular disease.

Molecular specificity and selectivity

There are different types (isoforms) of PDE in our bodies. They differ in terms of location and function. While other medications are not selective and act on all types of PDE, Sildenafil acts specifically on PDE5, leaving the other isoforms untouched. This peculiarity ensures high efficacy and minimizes unwanted side effects related to non-target tissues.

Pharmacokinetics and Metabolism

The drug's mechanism of action is strongly influenced by its absorption and metabolism:

  1. Absorption  . The medication is rapidly absorbed. Its maximum plasma concentration is reached 30 to 120 minutes after administration if taken on an empty stomach. Note that if you eat fatty meals, the absorption process of the active ingredient will be slowed, reducing the maximum plasma concentration.

  2. Metabolism.  Sildenafil metabolism occurs in the liver. The cytochrome P450 enzymes CYP3A4 and CYP2C9 participate in the process.

  3. Elimination.  The half-life of the ingredient is approximately 4 hours. It is excreted in both feces and urine. Most of the component (80%) is excreted in the feces, while only 13% of Sildenafil is eliminated from the body through urine.

Expanded mechanisms and more comprehensive benefits

In addition to its primary impact on male erectile function, the ingredient also affects muscles and blood vessels.

  1. Improved endothelial function.  Sildenafil improves the regulation of capillary blood flow by dynamically altering the constriction and dilation phases of blood vessels. This effect is achieved due to its ability to increase NO availability and cGMP levels. This action helps reduce oxidative stress and maintain vascular integrity.

  2. It improves performance during workouts.  The ability to improve blood circulation and promote vasodilation allows the ingredient to enhance physical performance. This action delays fatigue and increases endurance.

  3. Possible improvement in muscle performance.  The ability to improve blood circulation and oxygen supply makes Sildenafil beneficial for bodybuilders and sports enthusiasts. These characteristics increase endurance, reduce fatigue, and accelerate recovery.
