Oxymetholone (also known as Anadrol or Anapolon) is a C17-alpha-alkylated modification of dihydrotestosterone, effective orally, and considered by many to be one of the most potent commercially available steroids. Launched in the United States as a prescription drug in the early 1960s under the famous brand name Anadrol-50, oxymetholone is a very potent steroid developed for the treatment of osteoporosis and anemia, as well as to stimulate muscle mass gain in malnourished and debilitated patients. The hydroxymethylene group at the C2 carbon position is a unique structural feature of oxymetholone that considerably increases its anabolic effects and biological activity, resulting in significantly greater anabolic activity than testosterone. Oxymetholone causes rapid and significant gains in muscle mass, as well as a significant increase in strength and power. Furthermore, it is known to promote water retention, which can lead to increased blood pressure during the cycle. Due to its ability to increase hemoglobin levels and blood volume in the body, oxymetholone can cause an extremely strong muscle pumping effect, which consequently can complicate training, as some users experience such an intense pump that their muscles may become sore after the first set with heavy weights.

It is suggested that the estrogenic activity of oxymetholone may be related to a progestogenic effect similar to that of nandrolones; however, although the side effects may appear similar, clinical studies have shown that oxymetholone does not have any progestogenic activity.
